What to bring to a prearrangement?

Before going to a prearrangement appointment for funeral services, it can be helpful to write down the following information: 

  • Your full name, birthday, and where you were born
  • Your parents' full name, including your mother's maiden name
  • Your social security number
  • Your highest level of education
  • How you would best describe your career title
  • Marital status and name of spouse if applicable
  • If you were in the military, locate in your discharge paperwork. 

This information is used to create a death certificate once the prearrangement is needed at time of death. It also ensures that your family will not need to guess on information such as what you would consider your career. 

Before you attend the prearrangement appointment, here are some questions to consider and items to locate:

  • Do you prefer burial or cremation?
  • Where do you want your final resting place to be?
  • Do you already have arrangements with a cemetery or mausoleum?
  • Are there any religious practices you would want followed at the service?
  • Life insurance policy
  • Copy of a will or trust
  • Military discharge paperwork, if applicable 
  • Death benefit insurance, if applicable
